5. Amendment of outstanding SAR resulting in deemed cancellation of SAR and re-grant of replacement SAR. The SAR was originally granted on March 3, 2010 and was amended to extend the post-termination exercise period from 3 months to 3 years.

6. Amendment of outstanding SAR resulting in deemed cancellation of SAR and re-grant of replacement SAR. The SAR was originally granted on March 3, 2011 and was amended to extend the post-termination exercise period from 3 months to 3 years.
