Effective May 15, 1998, Ronald A. Lefkon resigned his position as Chief
Financial Officer of Continental Choice Care, Inc. ("Registrant").
Mark N. Raab, the Registrant's controller, has been appointed acting
Chief Financial Officer of the Registrant.
