2. These stock options were received under the Issuer's 2008 Stock Option and Incentive Plan and became fully exercisable on 12/21/2012.

3. The original report attributed the stock options as granted under the Company's 2008 Stock Option and Incentive Plan. This Report was revised to reflect that the stock options were granted under the Company's 2000 Stock Option and Incentive Plan. This correction relates to eight subsequent reports filed by the Reporting Person.

4. These stock options became fully exercisable on 12/31/2005.
