1. The shares were to vest as follows: 6,608 shares on 10/16/19 and 6,608 shares on 10/16/20. On November 22, 2019, the vesting schedule was revised so that the shares vest as follows: 4,405 shares on 10/16/19 and 8,811 shares on 10/16/20.

2. The options vested as follows: 6,667 on 6/18/15; 6,667 on 6/18/16; and 6,666 on 6/18/17.
