On June 30, 2015, Len DeBenedictis, age 74, ZELTIQ Aesthetics, Inc.’s Chief Technology Officer, requested, and on July 1, 2015, ZELTIQ agreed, that effective July 1, 2015, Mr. DeBenedictis would reduce his work hours commitment with ZELTIQ by 20%, and commensurate with that reduction that his salary would be reduced by 20%. This change does not reflect a change in his duties or responsibilities, his title, or any other terms of his employment.
