1. Represents shares purchased by the reporting person at the public offering price of $25.00 per share in connection with the issuer's underwritten public offering of common stock, which closed on September 30, 2024.

2. Includes 74.833 Dividend Equivalents credited to unvested time-based RSUs in lieu of a cash dividend payment since the reporting person's last required Form filing. Each whole Dividend Equivalent is the economic equivalent of one share of MidWestOne Financial Group, Inc. common stock.

3. Shares allocated to the reporting person's account from within the MidWestOne Financial Group, Inc. Employee Stock Ownership Plan, as of September 25, 2024.
