On May 3, 2016, the reporting person received a grant of 10,000 shares of restricted common stock pursuant to the Quorum Health Corporation 2016 Stock Award Plan. 100% of the number of shares of restricted stock issued shall vest, and the restrictions with respect to such restricted stock shall lapse, on the first (1st) anniversary of the date of the grant.
