(1) Consists of (i) 1,750 shares of Common Stock held directly by Bobak Azamian, (ii) 1,233,271 shares of Common Stock held by Bobak Azamian as Trustee of the Bobak Azamian Living Trust, established April 16, 2018, and (iii) 680,274 shares of Common Stock issuable upon exercise of options exercisable within 60 days of December 31, 2020 held directly by Dr. Azamian. Dr. Azamian is a trustee the Bobak Azamian Living Trust, established April 16, 2018 and has voting and investment control with respect to these shares.
