A description of the 7.375% Notes due 2051 (the “Notes”) of Qwest Corporation (the “Company”) is contained in the Prospectus included in the Registration Statement on Form S-3 (Registration No. 333-156101), which was filed by the Company and certain of its affiliates as an automatic shelf registration statement and was effective upon its filing with the Securities and Exchange Commission on December 21, 2008, as supplemented by a Preliminary Prospectus Supplement dated May 31, 2011, and a Prospectus Supplement dated June 1, 2011, each relating to the Notes and filed pursuant to Rule 424(b) under the Securities Act of 1933.
