(the "Company") to the reporting person pursuant to the Sanara MedTech Inc. 2014 Omnibus Long-Term Incentive Plan, as amended and restated. The shares of restricted stock will vest on June 10, 2023, provided that the reporting person is providing certain services to the Company on such date.
