ITEM 5.02 DEPARTURE OF DIRECTORS OR PRINCIPAL OFFICERS; ELECTION OF DIRECTORS; APPOINTMENT OF PRINCIPAL OFFICERS
On September 13, 2004, Universal Hospital Services, Inc. (“UHS”) appointed Andy Weaver, age 40, as its Controller and Chief Accounting Officer. Mr. Weaver’s first day of employment with UHS will be September 27, 2004. Prior to joining UHS, Mr. Weaver had a variety of accounting, corporate finance and treasury financial management roles at Reliant Resources, The Coca-Cola Company and The Minute Maid Company. Mr. Weaver has a Bachelor of Business Administration degree and a Master’s degree in Finance from Texas A&M University, and is a CPA.
